Russian Speaking Social Worker

The L’Chaim Adult Day Health Center is a licensed facility and outpatient center that provides medical, social, and therapy services for Russian-speaking older adults and individuals with disabilities. We have been a haven of person-centered care and community support for thousands of emigre clients from San Francisco and San Mateo for over 25 years.

POSITION SUMMARY:

Under the supervision of the Adult Day Health Care Program Director, the full time Russian Speaking Social Worker’s responsibilities include facilitation and oversight of assessments, care plans, direct services to clients, and charting responsibilities for the Russian Adult Day Health Care program. The Russian Speaking Social Worker understands the Agency’s mission, goals and objectives, and works within the Agency’s resources.

COMPENSATION AND BENEFITS:

• Pay: $90,000
• Employment Type: Full-time, exempt position with benefits

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

• Facilitates initial evaluation and 6-month assessments and quarterly reports for clients attending the center
• Formulates the social service component of the Individual Plan of Care and Treatment Authorization Requests for assigned participants in compliance with ADHC standards for establishing medical necessity and providing appropriate care
• Ensures that social service care plans are coordinated with nursing, physical therapy, and occupational therapy care plans, and fully address the social service needs of clients within the scope of work of a licensed Adult Day Health Care
• Provides case management to participants as recommended in their treatment plans, or as needed
• Discharges participants if they are inappropriate for ADHC services; assists caregiver with discharge plan
• Performs other documentation duties including charting in progress notes
• The Social Worker will work the hours necessary to meet the needs of the participants based upon their plans of care and to meet the requirements specified in Title 22, California Code of Regulations, Sections 54329 &78339
• Provides appropriate information and referral for community members and for all ADHC participants when needed
• Provides referral for persons not appropriate for adult day health care program
• Reports suspected verbal, physical, fiduciary, and/or sexual abuse to appropriate monitoring agencies
• Maintains case records, performs case follow-ups, and closings, and performs other administrative tasks as assigned in compliance with Agency standards, including JFCS’s Policy and Procedures Manual

JOB QUALIFICATIONS:

• Russian speaking is required
• Bachelor’s degree in social work from an accredited school with two years of experience providing social services in one or more of the fields of aging, health, or long-term care services or master’s degree preferred
• Demonstrated expertise (minimum of two years experience) in clinical casework with the elderly Russian-speaking Émigré population
• Knowledgeable about the physical, emotional, and social needs of older adults
• Proven experience assessing the social service needs of frail elderly and developing care plans to address needs in a manner that enables measurement of outcomes
• Proven ability to document care plans, interventions, and outcomes in an effective manner that meets professional standards for documentation of social services
• Proven ability to provide effective case management services to frail elders
• Experience providing services in an Adult Day Health Care or comparable environment
• Excellent oral and written communication skills in Russian and English required
• Ability to communicate appropriately and effectively with participants, professional staff, families, and caregivers
• Ability to effectively utilize Microsoft Office Suite software including Outlook
• Physically capable of performing the required duties and of assisting frail older adults and disabled adults, as necessary
